Proposition of a new organizational form between the city and the hospital in the field of therapeutic education: cooperative outsourcing

Résumé

Introduction : La loi santé 2016-41 du 26 janvier 2016 prescrit, en tant qu’élément central du système de santé français, la coordination des soins. Dans le champ de l’éducation thérapeutique du patient (ETP), le but est de renforcer sa continuité entre l’hôpital et la ville en associant tous lieux d’opportunités complémentaires au lieu habituel et majoritaire de l’éducation que représente l’hôpital. Afin d’explorer une hypothèse d’externalisation de l’ETP de l’hôpital par l’hôpital vers la ville avec l’intention de développer une culture du décloisonnement, il est nécessaire d’assurer la qualité de la relation entre les différents acteurs concernés. Objectifs : L’objectif de cette étude est de faire émerger des attentes et conditions vis-à-vis d’une telle hypothèse par les acteurs concernés. Méthodes : Une étude qualitative et inductive, de type exploratoire et propositionnelle a été menée de janvier à mars 2017 sur un échantillon de 26 acteurs de l’éducation thérapeutique en ville comme à l’hôpital, patients inclus. Résultats : Si la relation ville/hôpital est vécue comme cloisonnée et concurrentielle, la complémentarité de ces deux systèmes est souhaitée et pensée de manière organisée autour de l’externalisation d’une expertise hospitalière. Discussion : Les résultats amènent à penser cette externalisation inclusive et coopérative entre ces mêmes acteurs. Conclusion : Décloisonner l’hôpital vers la ville est possible et indispensable, en particulier en ETP. Néanmoins, il convient de définir un cadre à cette collaboration ville-hôpital dont l’externalisation coopérative pourrait être une proposition.

Abstract

Introduction: The healthcare law 2016-41 from 26 January 2016 prescribes, as the central element of the French healthcare system, the coordination of care. In the field of therapeutic patient education (TPE), the aim is to strengthen its continuity between the hospital and the city, by associating all complementary opportunities with the usual and majority place of education that the hospital represents. In order to explore an hypothesis of outsourcing of the hospital’s ETP from the hospital to the city with the intention of developing a culture of decompartmentalization, it is necessary to ensure the quality of the relationship between the different parties concerned. Objectives: The objective of this study is to raise expectations and conditions with respect to such a hypothesis. Methods: A qualitative and inductive study, exploratory and propositional, was conducted from January to March 2017 on a sample of 26 therapeutic education actors in the city and hospital, including the patient. Results: If the city/hospital relationship is experienced as compartmentalized and competitive, the complementarity of these two systems is desired and structured in an organized way around the outsourcing of hospital expertise. Discussion: The results lead us to think that this outsourcing requires cooperation. Conclusion: Decompartmentalizing the hospital towards the city is possible and indispensable, especially in FTE. Nevertheless, it is necessary to define a framework for this collaboration between the city and the hospital, whose cooperative outsourcing could be a proposal.
