Un jardin thérapeutique pour l’éducation thérapeutique du patient

Résumé

Introduction : L’éducation thérapeutique du patient est une approche humaniste où l’environnement de la nature n’a jamais été intégré. Objectifs : Évaluer qualitativement l’impact d’un jardin thérapeutique dans un programme interdisciplinaire d’éducation thérapeutique pour patients obèses. Méthode : Seize personnes randomisées souffrant d’obésité ont suivi un programme d’éducation thérapeutique de perte de poids durant deux ans. Elles ont aussi bénéficié de six ateliers avec une hortithérapeute. Seize patients contrôles randomisés ont suivi le programme habituel sans les ateliers du jardin. L’évaluation a été faite après une année et comparée avec ce groupe contrôle. Résultats : Six patients obèses ont mentionné à de nombreuses reprises que ces ateliers dans le jardin ont favorisé leur motivation à prendre soin d’eux-mêmes. Pour neuf patients, ces ateliers ont permis d’améliorer le partage entre eux. La perte de poids des patients est significativement meilleure (p < 0,004) dans le groupe qui a bénéficié des ateliers dans le jardin. Discussion : Le jardin permet de créer un lieu convivial d’apprentissage et de partage social. Il favorise de nouvelles attitudes d’échange entre les patients eux-mêmes mais aussi avec les soignants. Conclusion : Une éducation thérapeutique du patient incluant la nature peut compléter très favorablement un programme interdisciplinaire pour patients obèses. Les résultats de cette recherche mériteraient confirmation par des études prospectives contrôlées sur un plus grand nombre de patients.

Abstract

Introduction: Therapeutic patient education is a humanistic approach into which nature was never integrated. Goals: Evaluate the impact of a therapeutic garden in an interdisciplinary programme of therapeutic education for obese patients. Method: sixteen randomized people suffering from obesity have followed a weight loss therapeutic education programme during 2 years. They also had 6 workshops with a hortitherapist. Sixteen control randomized patients followed the usual programme without the garden workshops. Evaluation was made after 1 year and compared with this control group. Results: Six obese patients mentioned several times that these garden workshops motivated them to take better care of themselves. For nine of them, it helped them to share with others. Weight loss is significantly better (p < 0.004) in the group which benefited from the garden workshops. Discussion: The garden helps to create a user-friendly learning and sharing place. It promotes new sharing attitudes between patients but also with healthcare providers. Conclusion: A patient therapeutic education including nature can very positively complement an interdisciplinary programme for obese patients. Results of this research should be confirmed by controlled prospective studies with a larger number of patients.
